I am aware that, as a participant in the event, it will be necessary for to make certain of my personal information available to service providers and suppliers of the event and for this purpose:

• I acknowledge that my personal information is protected by the Protection of Personal Information Act, 4 of 2013 and that my personal information may not be made available in any manner or form, without my consent and which consent I may withdraw at any time;
• I herewith consent thereto that may make my personal information available to all the service providers and suppliers of the event, for purposes of the event, and;
• Should I at any time withdraw my consent (which shall be in writing with notice to ), I shall immediately disqualify myself from participating in and attending the event.
